Build Application and Feedback Forms

Last updated: October 8, 2026

At Maple Hall… Jordan Lee writes one application form per job but a single review form that every reviewer in the department uses, so scores mean the same thing across jobs.

Recruitment uses two kinds of forms. Application forms are what candidates fill in when they apply to a job, and feedback forms are what reviewers and interviewers fill in about a candidate. You build both in the form builder under Recruitment → Forms. Application-form selectors in a job's workflow pick from recruitment forms, and feedback forms can be attached to the application-form and interview workflow items.

Build forms before you post a job. Forms start unpublished and must be published separately using the Publish action, and a form that already has applications is harder to change without affecting how responses compare.

Before you build

  • The Forms page needs the "View all HR forms" permission, and creating or editing forms needs "Create and edit all HR forms".

  • Decide which kind you need first: application forms for candidates, or feedback forms for reviewers and interviewers.

Find the two form kinds

The Forms page has two tabs. The Recruitment tab holds application forms, and the Feedback tab holds feedback forms. Click Add Form on the tab you want: it creates an Untitled Form of that kind and opens the form builder. Open Settings and set the Form Name so the form is easy to recognize when a job points to it.

Add and set up questions

Add Question opens the list of question types, including text, multiple choice, radio, dropdown, scale, date, date range, file, static text and signature. Static text adds instructions rather than a question. Each question gets a title and a Question Type, and you set whether it is Required or Optional. Use Duplicate to copy a question instead of rebuilding a similar one, and Preview to check how the form looks.

Group questions with sections

Use Add Section to split a long form into parts, and give each one a Section Name. Sections help candidates work through a long application and help reviewers find what they are being asked.

Reuse a form across jobs and rounds

Application-form selectors in a job's workflow pick from recruitment forms, and feedback forms can be attached to the application-form and interview workflow items, so you do not need a new form for every posting. One feedback form can serve every job and every interview round, which keeps reviewer scores comparable across the department. A form stays unpublished until you publish it with the Publish action.

Work with saved forms

Every saved form appears as a card on its tab. The card offers View Form and View All Responses, so you can review the questions and read submitted answers.

Change a form later

Edit a form before applications arrive. Changing questions after applications exist affects how responses compare, because applicants answered the form as it was when they applied.

After you save

The form appears as a card on the tab where you created it. Point a job step at it when you post the job, and publish the form with the Publish action. View Form opens the form editor, and Preview shows the form as the person filling it in will see it.
