Add a Form to a Request Type
Last updated: October 8, 2026
Attach a Task form to a request type to ask residents exactly the questions each type needs, instead of relying on a free-text description. Residents answer those questions, including uploading a photo, when they file the request, and staff read the answers on the task page. Only forms with Form Type Task can be attached to a request type. You choose the form type when the form is first published, and it cannot be changed afterwards.
Before you start
You need a request type to attach the form to. If your school has not set one up yet, start with Set Up Request Types (Task Categories). Build the form itself in Setup → Forms; the form builder lives there, not in the Support screens. You need the permission to manage forms to build the form, and the Ability to configure task settings permission to attach it to a request type.
Build the form
Go to Setup → Forms and create a new form. Set Form Type to Task. A Task form can use multiple sections, so organize the questions across sections however your staff need them. Publish the form before attaching it: a form that has not been published cannot be attached to a request type.
Add a question for each piece of information staff ask for again and again. A file question lets residents attach a photo,
Attach the form to the request type
Open the request type's card and click the pencil to edit it. Find Custom Form Policy, described as "Assign custom Forms to Task Categories to collect the most relevant information", and pick your Task form from the list. Click Save.
The saved card shows the form's name on the request type, so anyone setting up types can see which form a type uses.
What residents see
When a resident files a request of this type, the form's questions appear under Form questions on the Details step. A question of file type lets them add a photo before they send the request. Required questions are checked before the request can be sent.
The Description box
A type with a form has no Description box. The form replaces free text entirely, so residents can only write what your questions ask for.
If you still want residents to add anything else, put an "anything else?" question on the form. Without one, details that fall outside your questions have nowhere to go.
Where answers show for staff
On the task page, the resident's answers appear on the form answers card. Photos that residents uploaded also appear under Images, so staff see the picture without opening the answers card.
Changing the form later
Editing the form does not change answers already given. Requests filed before your edit keep the answers residents gave, but the question text on existing tasks shows the current version, so edited and republished questions may read differently than when the answers were filed. If you remove a question that already has answers, the answers are not lost: the task page keeps them under Other Responses.
Good to know
A type with a form has no Description box. Add an "anything else?" question to the form if you want free text.
